Standard Daycare Policies and Group Play Consent

  Dogwood Veterinary Hospital & Pet Resort will perform a temperament test on all dogs prior to introduction into Dogwood Daycare. The temperament test allows the opportunity for a dog to be slowly introduced to group play while under the close supervision of our staff. We will review the information provided by you and use that information in conjunction with the temperament test to determine your dog’s suitability for group play or Daycare. We will make every effort to place dogs in appropriate groups based on temperament, energy level, and age. Dogwood Veterinary Hospital & Pet Resort reserves the right to refuse group play to your dog if any aggressive behavior is exhibited towards another dog or human. Dogs, like people, may have a bad day or not get along with certain dogs. If this is the case, we may remove your dog from the group for a “break.”

 All vaccinations must be current. Proof of Rabies, DHPP, Canine Influenza, Bordetella vaccines, and Fecal test or deworming (bi-annual) must be provided prior to admittance into Dogwood Daycare. If your pet is not current, vaccines and bi-annual parasite recommendations will be administered at the first visit. All guests will be screened for fleas and ticks. If external parasites are noted, they will be treated appropriately, and charged at regular rates.

 All dogs over the age of twelve months must be spayed or neutered in order to participate in group play or Daycare.

 Until fully vaccinated against Distemper, Rabies, Canine Influenza and Bordetella (for puppies), puppies and under the age of 16 weeks will not be allowed to participate in group play or Daycare.

 Dogwood Veterinary Hospital & Pet Resort will practice diligence to ensure the safety, wellbeing, and happiness of every dog or in Daycare. However, there are certain risks associated with allowing the freedom of dogs interacting with other dogs. Even with constant supervision, nips, scratches, muscle soreness, disease, and dog fights may occur. Each pet owner needs to understand the risks involved.

 By signing below, I understand and agree to the terms stated above. I understand the risks associated, I authorize Dogwood Veterinary Hospital & Pet Resort to medically treat my dog if necessary, and I agree to pay for any and all fees incurred. In addition, by signing below, I release Dogwood Veterinary Hospital & Pet Resort, its staff, doctors, agents, and all affiliates from any and all liability for any injury or damage that may arise from, but not limited to, actions from another dog during group play or Daycare.

Dogwood Veterinary Hospital & Pet Resort prides itself on providing a safe, happy, and comfortable place for our canine guests and their owners. We enjoy and strive to keep open and honest communication with all of our clients and will let you know how your dog is doing. In addition, we are always open to suggestions on how we may make your and your dog’s visit with us even better. Please do not ever hesitate to ask if you have any questions regarding your pet’s stay with us. This agreement and its terms bond a relationship between Dogwood Veterinary Hospital & Pet Resort and yourself. Each time your dog enters Dogwood Veterinary Hospital & Pet Resort, you affirm the terms of this agreement and agree to indemnify and hold Dogwood Veterinary Hospital & Pet Resort and its staff, doctors, agents, and all affiliates harmless from all liability and claims. Please note that we provide food and water bowls for all pets during their stay with us. We also offer pet cots and fresh bedding. You are welcome to bring some of the comforts of home and we will gladly place these items in your pet's suite with them. We sometimes have to remove bedding and toys if they become soiled. Though we use care in handling your pet's belongings and we label and store them appropriately, we cannot guarantee that these items will not be lost or damaged during your pet's stay.

Treatment Release and Emergency Consent

It is our goal to provide your pet with the best care possible in any circumstance. If an emergency arises or a procedure is necessary while your pet is staying with us, we will make every possible effort to contact you. In case we are unable to reach you, we ask that you sign this release form. Your authorization on this release will allow for us to provide veterinary care in your absence. Please let us know if you authorize anyone else to make these important decisions on your behalf.

Dogwood Requirements

Canine Guests must have current Rabies, DHPP, Canine Influenza and Bordetella vaccines, and biannual fecal test or deworming.
Feline Guests must have current Rabies and FVCRP vaccines, and biannual fecal test or deworming.
If your pet is not up to date, vaccines and parasite requirements will be administered. All guests will be screened for fleas and ticks prior to their stay and treated as necessary (at additional cost).
Dogwood Veterinary Hospital & Pet Resort requires a minimum of one night’s deposit for all reservations; three night’s deposit during holidays.
